Important Info for Back to School

Immunizations:  All proof of shots need to be ON FILE no later than August 23, 2021 to avoid exclusion from classroom placement.

Medications:  If your child requires medication to be given during the school day, please bring in all appropriate paper work and medication to the nurse’s office. 

Attendance:  If your child is ill, DO NOT send him/her to school. If you have questions about when they can return call your student’s building nurse

Toileting:  If your child is prone to wetting or soiling accidents, please send an extra pair of clothing in the backpack.

Required Vaccine

Reminder emails will go out several times this year to ALL Juniors. If your student has already had the vaccine and turned paperwork in disregard the email.  

We are informing parents of all juniors that the Meningococcal Vaccine is required prior to the beginning of your student’s senior year. 

Your student will not be able to start school next August until record of the meningococcal vaccine is obtained. 
I suggest getting the vaccine and vaccine record turned in as soon as possible.

Prescription Medication

Parents please be aware that if a student absolutely needs to take medication at school they will need doctor orders for the medication.  The orders must be for the student, signed by the doctor and by the parent. Medication must be in the original medication bottle. Parents will need to bring the medication to school & deliver it to Nurse Hobbs.  Medication isn't to be sent with the student or left in the office. Please call ahead to schedule a time to
drop off medication with our Nurse.

 

Standing Orders & Health Services

The Lincoln County R-III School District has worked with Dr. James Bockhorst to establish medical standing orders to service our students more efficiently.  Parents that sign this permission form are giving our school nurse and health service professionals authority to administer medication.  Please click this link to view and print our Standing Orders & Health Inventory forms.
