NAME CHANGE INFORMATION
Prior to changing your name on any R-III programs, including payroll/benefits, the District requires that you complete the following steps:
(1) Provide Human Resources with an updated Social Security card and Drivers License reflecting your name change.
(2) Submit a name change online form through Skyward.
(3) Update your Teaching or Substitute Certificate from DESE, reflecting your name change.
Should you need to add or drop dependents on your insurance, you must contact HR within 30 days of the event date.

TEACHER CERTIFICATION – DESE
You will be required to complete an Update Personal/Education Information Request Form to make a change. Please print, complete, and sign a copy of this cover sheet and mail it with one form of supporting documentation. Acceptable supporting documentation includes the following: copy of driver’s license, Social Security card, marriage certificate, divorce decree, or birth certificate.
