Supplemental Insurance Policies - Unum

Unum handles the following policies for the District: Basic Life, Voluntary Life, Whole Life, Long Term Disability, Short Term Disability, Critical Illness, Hospital Indemnity, and Accident.  


Current policies will remain in force until 12/31/2024.  Open enrollment for Unum policies will be held in the fall of 2024 for the 2025 calendar year. Visit https://www.unum.com/employees for plan details and to make a claim. 

 

For Basic Life, Voluntary Life, and Long Term Disability questions and claims, please call: 1.800.633.7479.

 

For Whole Life Short Term Disability, Critical Illness, Hospital Indemnity, and Accident questions and claims, please call: 1.800.635.5597.   

 

403(b)

A 403(b) plan (tax-sheltered annuity plan or TSA) is a retirement plan available to public school employees. This is a voluntary retirement funding option and is in addition to the required funding of your PSRS/PEERS account. LCR3 partners with OMNI to manage 403(b) opportunities for our employees.

Employee Assistance Program (EAP)

UHC and Unum know that your physical health is not the only thing that needs a little assistance sometimes.  That’s why they offer their Employee Assistance Programs (EAP). 

 

United Healthcare

United Healthcare's EAP offers help with a range of issues at no charge for members and families.  Some of the services included are:

Unum - Work Life Balance

Unum's EAP is called Work Life Balance and offers a  variety of resources to aid in dealing with questions, concerns, and emotional issues surrounding your personal or work life.  You can access these resources on the telephone and online, 1-800-854-1446 or unum.com/livebalance

National Benefit Services - 125 Plan and FSA

National Benefit Services (NBS) handles the Districts' Section 125 plan and our Flexible Spending Accounts.  On this page you will find contact information for NBS as well as forms and plan documents.
